A 58-year-old male with stable angina pectoris CCS class II presents with hypertension, type 2 diabetes mellitus, dyslipidemia, and normal kidney function (BP 154/88 mmHg, HbA1C 7.8%, TC 182 mg/dl, LDL-C 108 mg/dl, HDL-C 28 mg/dl, triglycerides 324 mg/dl, and serum creatinine 0.9 mg/dl). He denies family history of premature cardiovascular disease.
